The Physical Therapist will be responsible for reviewing physician referrals, evaluating patient function, and developing and modifying treatment plans to restore mobility, strength, and function. Key duties also include administering therapy, educating patients and families, completing Medicare documentation, and collaborating with care teams.
Candidates must possess a Bachelor's degree in Physical Therapy and hold a current state PT license. Experience in Long-Term Care settings and strong knowledge of Medicare guidelines are preferred qualifications.
Physical Therapist (PT) - Long-Term Care LP Therapy LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
review physician referrals and medical records; evaluate strength, mobility, and functional ability; develop and modify treatment plans; administer therapeutic exercises and modalities; educate patients and families; complete timely Medicare documentation; collaborate with care teams; supervise PTAs and aides as required; support quality outcomes and productivity standards.
Bachelor's degree in Physical Therapy; current state PT license; LTC experience preferred; strong knowledge of Medicare guidelines; effective communication and teamwork skills. Work Environment: LTC facility; physical demands include lifting and mobility assistance.
