The provider will deliver primary care services to a dedicated patient panel, which includes conducting physical exams, interpreting diagnostic tests, and developing comprehensive treatment plans. Responsibilities also involve establishing long-term patient relationships while focusing on health promotion, disease prevention, and practicing evidence-based, holistic care.
Candidates must possess current Nurse Practitioner (NP) or Physician Assistant (PA) licensure and ideally have experience in primary care or family medicine. Essential qualities include a passion for patient-centered care, strong clinical and communication skills, and the ability to be independent, self-motivated, and capable of self-scheduling.

· Provide primary care services to a dedicated panel of patients · Conduct physical exams, order and interpret diagnostic tests, and develop treatment plans · Establish and maintain long-term patient relationships · Offer health promotion, disease prevention, and education · Work collaboratively with other providers in a supportive, team-oriented environment · Practice evidence-based medicine with a focus on holistic and preventative care · Work collaboratively with the marketing team to grow our practice and serve more patients
· Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure · Experience in primary care or family medicine preferred · Passion for partnering with your patients and patient-centered care · Ability to build trust and strong relationships with patients · Excellent communication and clinical skills · Independent, self-motivated, and proactive approach to care · Ability to self-schedule and to prioritize patient needs · Home health or hospice experience is encouraged
