The physical therapist partners with patients and families to perform evaluations, develop individualized plans of care, and implement skilled interventions. They also coordinate care with the interdisciplinary team and establish appropriate discharge plans.
Candidates must be a licensed Physical Therapist in Minnesota or have a temporary license, along with Basic Life Support certification. A Doctorate of Physical Therapy and one year of experience are preferred qualifications.
The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation Gathers pertinent data Performs examination Determines treatment diagnosis Develops an individualized plan of care through collaboration with the patient and family Analyzes data to identify needs and potential risks Determines prognosis for recovery Establishes individualized goals with patient and/or caregiver Individualizes plan of care considering developmental level and cultural differences Implements and modifies plan of care based on reassessment and patient response Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care Establishes appropriate discharge plan Partners with patients and families to provide skilled intervention Implements plan of care Alters treatment/plan of care to reflect change in patient status and response to treatment Assesses patient and/or caregiver learning needs and provides appropriate education
Required Qualifications Physical Therapist in the state of Minnesota or temporary license in the state of Minnesota Basic Life Support (American Heart Association or Red Cross)
Preferred Qualifications: Doctorate of Physical Therapy 1 year experience
