The Social Worker will evaluate complex patient and family situations involving legal, ethical, and psychosocial components within a fast-paced urgent care setting. Responsibilities include developing patient goals, providing brief interventions, addressing immediate barriers, and coordinating safe care transitions or follow-up.
A Master's Degree in Social Work is required for this position. Preferred qualifications include 2 years of prior hospital-based social work experience, and candidates must obtain a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) certification in Wisconsin within 90 days.
This is a full time, 100% FTE position, day/evening. Monday – Friday between the hours of 12:00pm – 8:00m, no weekends required. Hours may vary based on the operational needs of the department.
This position will provide primary onsite coverage for Union Corners Urgent Care daily, with remote support provided to West Towne Urgent Care.
