The Case Manager provides comprehensive support through in-home and/or community-based intervention to adults with Serious Mental Illness, including performing assessments, developing service plans, and coordinating care with the member's team. Key duties involve monitoring mental status, providing crisis intervention, educating members and support systems, and maintaining accurate documentation of progress and services.
A minimum requirement is a Bachelor's degree with one year of full-time behavioral health experience, or an equivalent combination totaling five years of relevant education and experience. Candidates must also obtain fingerprint clearance from the Arizona Department of Public Safety and possess a valid Arizona driver's license with personal vehicle use for business needs.
