The primary purpose of this role is to ensure regulatory compliance and foster a person-centered culture within the 34-bed Rehab unit by providing clinical expertise and facilitating the development of patient-specific care plans. Responsibilities include coordinating daily assignments, overseeing clinical records maintenance, conducting nursing assessments, and collaborating with the Interdisciplinary Team (IDT) on patient care plans.
Candidates must possess a current and valid New York State Registered Nurse (NYS RN) license. A minimum of two years of experience as an RN working specifically with geriatric patients is required, along with existing knowledge of community resources and clinical organizational structures.

Participates in the household team by providing clinical expertise. Facilitates the ongoing development of Elder/patient specific care plans. Regularly communicates to all team members any changes in these care plans. Ensures professional standards of nursing care. Institutes best practices and optimal quality of care within the household setting are met. Coordinates daily assignments assuring care is provided according to established policy and procedure. Oversees maintenance of Elder/patient’s clinical records, reflecting current status and maintains confidentiality of all Elder/patient care information. May accompany Physician/Nurse Practitioner on rounds and keeps staff informed of orders concerning Elder/patients as assigned. Collaborates with and develops Elder/patients care plan with the IDT team. Reviews nursing documentation for accuracy and completeness re: Elder/patient care and response to care as necessary, utilizes nursing process to ensure best outcomes. Assists with diagnostic procedures as assigned. Transcribes medical orders. Admits and discharges Elder/patients in collaboration with nurse leaders. Completes Elder/patient and family rounding, while practicing effective communication and follow-up. Completes incident reports and investigates concerns with notification to Nurse Leaders. Conducts nursing assessments, collection of relevant information to serve as the basis of a nursing diagnosis, including (but not limited to): nutrition, elimination, hydration, skin integrity, respiratory and cardiac Communicates with Medical staff and family regarding significant change in status.
