The Social Worker provides supportive social work services to healthcare and rehabilitation residents to help them achieve their highest level of function, joy, and purpose. This role involves identifying and addressing residents' social, emotional, and psychological needs while ensuring compliance with professional standards and regulations.
The role requires performing professional social service duties, including assisting with admissions/discharges, participating in MDS-guided care planning, and maintaining accurate documentation per federal and state regulations. Essential duties also involve coordinating resources, communicating changes to families, and acting as the designated resident advocate.
Description The Social Worker provides supportive social work services to health care and rehabilitation residents while enabling them to attain and maintain the highest level of function which will bring joy and purpose to their lives. Performs a professional social service role in a manner that meets professional standards and complies with federal and state regulations. Works collaboratively to support the Mission and Values of SCBMA.

Identify and provide for residents’ social, emotional and psychological needs and aid in development of the resident’s full potential Assist in resident admission and discharge, as needed Participate in care planning, utilizing the minimum data set (MDS) to guide the care plan Maintain accurate and timely documentation complying with federal and state regulations and community procedures including MDS assessments, social service progress notes and care plans Coordinate services regarding resident care and resources for families Coordinate hospice referrals as needed Communicate with families and/or person responsible of resident changes such as transfer, room change or discharge Order needed equipment and/or provide resources on behalf of rehabilitation residents being discharged Assist in communicating with residents, families and/or responsible parties concerning health status or coping with facility placement, physical illness and disabilities of the resident, end of life and grieving process. Act as the designated resident advocate, recognizing the contribution of all staff members in this role Facilitate proper procedure on initiation/completion of grievance reports, documenting and following organization standards and regulatory requirements Supervise Assistant Social Worker, if needed Maintain Medicare certification/recertification notices in compliance with federal regulations Maintain a working knowledge of federal and state regulations, as related to the position Ensure compliance with all regulations from DHEC, Fire and Safety, OSHA, Labor Laws, etc and adhere to HIPAA confidentiality standards.
