The Physical Therapist identifies, prevents, remediates, and rehabilitates physical dysfunction or pain through evaluation and therapeutic application, exercise, and instruction. Responsibilities include performing comprehensive assessments, developing individualized plans of care in partnership with patients and caregivers, and communicating with physicians and other practitioners.
Candidates must possess a Bachelor's level degree and a minimum of one year of experience, preferably within a qualified home health agency. The role requires significant manual skills, motor coordination, finger dexterity, and involves some occupational risk and exposure to various physical environments.
Provides services that identify, prevent, remediate and rehabilitate acute or prolonged physical dysfunction or pain. Evaluates and analyzes patients and therapeutic application, physical and chemical agents, exercise, instruction to patient/family and other procedures. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any) and caregiver. Communicates with the physician who is responsible for the health plan of care and other health care practitioners as appropriate related to the current home health plan of care.
1. Performs physical therapy assessment including functional limitations, rooms, capabilities, mobility status, endurance status, ADL status. 2. Identifies patient and family needs for other home health services and refers for same when needed. 3. Provides services that are ordered by the physician as indicated in the plan of care. Communicates with the physician(s) involved in the plan of care and other health care providers as appropriate. 4. Provides appropriate treatment. Treats patients to relieve pain, develop and restore function and regain optimum level of independence using appropriate equipment and modalities. 5. Records on a visit basis, all evaluation data, treatments and client's response to therapy, provides client's discharge planning. 6. Participates in case management conferences with team members to coordinate physical therapy and total plan of care. 7. Provides counseling and education to patients, family and other health care personnel in the patient�s treatment regimen, and documents to whom this instruction has been given. 8. . Provides or arranges for equipment required to increase the client's function and independence. 9. Participates in the agency quality assurance and performance improvement program and the agency-sponsored inservice training. 10. Supervises care provided by the PTA(s) and home healt aides as appropriate. 11. Fulfills all compliance responsibilities related to the position. 12. Performs other duties as assigned.
