Position Summary
Lifestyle Coordinator planned and facilitated diverse resident activities to support engagement, wellness, and community involvement
Key Responsibilities
- Plan and implement activities utilizing the HEART philosophy
- Perform daily assessments of residents to determine programs according to individual plan of care
- Facilitate socialization among residents
- Promote activities in a way that piques residents’ interest by using the Staying Connected Form
- Provide input into each resident’s assistance plan
- Plan, coordinate and help implement decorations for special events, seasons and holidays
- Plan weekly out trips
- Identify and place requisitions from supervisor for needed supplies for planned activities
- Provide daily assistance during all meals
- Provide monitoring and direction to residents on the neighborhood to assist with maintaining their prior lifestyle
- Assist with transportation for activity outings and regularly scheduled group trips
- Carry out other duties as assigned by supervisor to ensure smooth operation of the program and to meet resident’s needs
- Assist with calendar preparation
Education/Experience/Licensure/Certification
- Experience in activity planning with seniors preferred
- Thorough knowledge of dementia related issues
- TRD certification, associate's degree, or additional training in related field preferred
- Minimum of 2 years' work-related experience in an environment which focuses on dementia care, preferred
- Evidence in creativity
- Demonstrated organizational skills
- Mature, pleasant, friendly, and cheerful personality
- Driver
- Requirements: Driver license required with clean driving record. May be required to take driving test
