The Case Counselor provides case management services, coordinating care and developing individualized service plans for clients. They collaborate with clinical staff, families, and community resources to ensure effective treatment and recovery progress.
A high school diploma and at least one year of professional experience in behavioral health or psychiatry are required. A bachelor's degree in social work and LADC licensure are strongly preferred.
Position Overview: The Case Counselor will provide case management services and work with the
clinical staff to coordinate care, educate on the continuum of care, and assist with treatment plans
for clients.
1. Participate in the assessments and developments of the service plan.
2. Implements services to patients in accordance with their individualized service plan (ISP).
3. Collaborates with the patient and patient’s family or significant others to implement an
effective service plan, explaining the available continuum of care and clinical options to the
team, including the advantages and disadvantages of each option.
4. Complete clinical intake documentation as assigned.
5. Complete clinical assessment for risk per policy.
6. Communicates and documents patient’s progress toward their recovery.
7. Maintains the patient’s comprehensive clinical record, including documentation of
activities performed as part of the service delivery process (e.g., assessments, provision of
services, coordination of care, discharge planning).
8. Works collaboratively with the clinical team to engage, educate, communicate, and
coordinate care with patient, their family, behavioral and general medical and dental health
care providers, community resources and others to ensure that all services prescribed in
the individualized service plan (ISP) are implemented.
9. Provides assistance in maintaining, monitoring and modifying covered behavioral health
services.
10. Works with all collateral contacts, reduce barriers to continuum of care engagement and
meeting goals for ISP.
11. Ensures all appropriate referrals for identified services on the service plan are made and
coordinated with service providers via collateral providers or community resources.
12. Participates in meetings regarding the continuum of care and discharge planning.
13. Work towards established goals and OKR’s for the case management department.
14. Other duties as assigned
High School Diploma and minimum of one (1) year of professional experience in Behavioral Health or Psychiatric is required Bachelor’s Degree in a social work field is preferred
Licensure is not required but a Licensed Drug and Alcohol Counselor (LADC) II or I strongly preferred
Knowledge of healthcare, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology. A working knowledge of Microsoft Word, Excel, and Outlook required
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.
