The Occupational Therapist will develop and implement personalized treatment plans, conduct evaluations using standardized tools, and provide therapy focused on activities of daily living, mobility, safety, and independence. Responsibilities also include educating patients and caregivers, maintaining accurate documentation, and collaborating with the multidisciplinary care team.
Candidates must possess a valid Occupational Therapist license in Colorado and have proven experience in home health or a related field. Excellent communication skills, proficiency in electronic medical record documentation, and a flexible approach to meeting diverse client needs are also required.
We are seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in Grand Junction, CO. You’ll help patients improve independence, mobility, and overall quality of life through skilled, client-centered care.
