The Licensed Vocational Nurse provides direct patient care according to the State's LVN Nurse Practice Act and implements the plan of care established by the registered nurse. Key duties include accurate documentation, patient and family education, and participating in the coordination of various home health services.
Candidates must be graduates of an accredited vocational nursing program and hold an active Licensed Vocational Nurse license in California, along with a valid California driver's license and proof of auto insurance. A minimum of one year of relevant nursing experience and current CPR certification are also required.

Provides direct patient care as defined in States LVN Nurse Practice Act. Implements a plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA,OT, PT, MSW, ST and Dietician) to the registered nurse and/or Clinical Supervisor/ Nursing Supervisor. Uses equipment and supplies effectively and efficiently. Participate in personal and progressional growth and development. Performs other duties as assigned by the registered nurse. Qualifications and Skills: Graduate of an accredited vocational nursing program. Active licensed vocational nurse in the state of CA. Valid California driver’s license and proof of auto insurance. At least one (1+) year of relevant nursing experience. Current CPR certification.
