The role involves providing value-based primary care services, focusing on personalized, preventative care, and spending quality time with senior patients to improve their quality of life. Providers must demonstrate compassion and kindness while managing chronic diseases within a value-based care model.
Candidates must possess at least 2-3 years of experience in primary care or value-based care settings, specifically working with Medicare Advantage, Medicare, and Medicaid populations. A strong understanding of chronic disease management and quality metrics, along with an active Florida APRN license and DEA eligibility, are mandatory.
