The Scheduler facilitates daily patient schedules through timely data entry of changes and notifies relevant partners and caregivers of these updates. This role also coordinates admissions, evaluations, service resumptions, and discharges with appropriate disciplines under the Director of Nursing's direction.
Candidates must possess a high school education or GED and a minimum of two years of experience in home care or a related healthcare role, along with computer proficiency. A Licensed Practical Nurse license and reliable transportation with a driver's license are preferred.
