The Physical Therapist will evaluate patients' functional limitations in home health settings and develop individualized treatment plans focused on restoring function, improving mobility, and alleviating pain. Essential duties include providing therapeutic interventions, educating patients and caregivers, monitoring progress, documenting care, and collaborating with the interdisciplinary team.
Candidates must possess an active Physical Therapist license and professional liability insurance specific to New Mexico, along with a graduate degree or higher in Physical Therapy from an accredited program. Experience in home health or rehabilitative settings is preferred, alongside strong adaptability and excellent communication skills.
