The Physical Therapist will evaluate patients' physical abilities and functional limitations in home health settings, developing and implementing individualized treatment plans to restore function and alleviate pain. Essential duties also include providing therapeutic interventions, educating patients and caregivers, monitoring progress, and accurately documenting all sessions using digital platforms.
Candidates must possess an active Physical Therapist license in Texas and professional liability insurance specific to that license, along with a graduate degree or higher in Physical Therapy from an accredited program. Experience in home health or rehabilitative settings is preferred, alongside strong adaptability and excellent communication skills.
