Evaluate patients' physical abilities and functional limitations in a home health setting, developing and implementing individualized treatment plans to restore function, improve mobility, and alleviate pain. This includes providing therapeutic interventions, educating patients and caregivers, monitoring progress, and documenting care.
Requires an active Physical Therapist license in New Mexico and professional liability insurance. A graduate degree or higher in Physical Therapy from an accredited program is necessary, with experience in home health or rehabilitative settings being preferred.
