The role involves performing Annual Wellness Visits, developing preventive care plans, and managing care for high-risk and ACO patients through coordination and participation in multidisciplinary huddles. Additionally, the nurse provides direct nursing care including triage, treatments, injections, labs, and patient admissions while maintaining compliance standards.
Candidates must possess a BLS Certificate from the American Red Cross or American Heart Association and hold a current Registered Nurse (RN) license in North Carolina. A minimum of one year of experience as an RN, LPN, or in a related medical field is required, along with a clean driving record and approved driving privileges.
