DCSNO is seeking full-time ECD Care Coordinator or Behavioral Health Care Coordinator.
This role is critical in ensuring that children and families receive the comprehensive care and support they need.
They will work collaboratively within the care team to ensure that children receive the best care and support.
They will serve as a trusted resource and advocate for families throughout the care process.
Their primary responsibility is to ensure that children receive the best possible care and support, particularly those with developmental or behavioral challenges.
The Care Coordinator position will work in partnership with families based on screening results, coordinate services by working closely with other healthcare and cross-sector providers, monitor progress, and advocate to ensure equitable access to the resources and services they need to support their child's development.
Duties & Responsibilities
Knowledgeable of and applies the Mission and Values of the DePaul Community Health Centers and Ascension Health Ministry.
Serves as a primary support staff to Pediatricians and LCSW.
Serves as a patients’ primary point of contact for case management or behavioral health services provided within the health center.
Works collaboratively with team members to ensure continuity of patient care.
Communicates with families via audio, video, and in person visits.
Reviews BRIEF and ASQ screeners provided by pediatricians and medical assistants for establishment of care coordination and/or behavioral health services.
Provides resources and referrals for families in need of SDOH or Behavioral Health Services.
Functions as the Liaison between referral providers and DCHC Providers to insure integration of care.
Documents all encounters with patients in the EMR system.
Coordinates with other disciplines as needed. Works within the team and with all other health center staff in the coordination of patient services.
Provides day-to-day support via phone consult or warm hand offs if applicable.
Reviews referrals and submits a monthly report of all patients referred and connected for case management or Behavioral Health Services.
Coordinates with LCSW to ensure patients follow up for behavioral health appointments.
Supports policies, procedures, and supervisory decisions within the Health Center.
Accepts supervision and feedback from Health Providers/Medical Director.
Participates in continuing education programs and meetings with partner agencies as requested.
Uses established channels of communication and proper lines of authority for problem solving and communication.
Travels to multiple clinic locations required.
Maintains an acceptable attendance record; provides proper notification for absences or tardiness; documents overtime including justification.
Attends DCHC general staff meetings, committee meetings and annual retreats and partner agencies.
Is knowledgeable of patients’ rights and responsibilities and accepts responsibility for the assurance of the same. Maintain and assure confidentiality in accordance with HIPAA Guidelines

Minimum Qualifications
Must have an BSW

Excellent organizational skills with the ability to pay attention to details.
