The Physical Therapist will evaluate patients' functional limitations in a home health setting and develop individualized treatment plans focused on restoring function, improving mobility, and alleviating pain. Essential duties include providing therapeutic interventions, educating patients and caregivers, monitoring progress, documenting care, and collaborating with the interdisciplinary team.
Candidates must possess an active Physical Therapist license in New Mexico and professional liability insurance specific to that license, along with a graduate degree or higher in Physical Therapy. Experience in home health or rehabilitative settings is preferred, alongside strong adaptability and up-to-date knowledge of evidence-based treatment techniques.
Active Physical Therapist license in New Mexico.
