The RN Case Manager is responsible for assuming primary responsibility for a patient caseload, which involves assessing, planning, implementing, and evaluating nursing care to maximize patient comfort and health. This role requires initiating communication with physicians and staff to coordinate optimal care and maintaining accurate and timely documentation in electronic medical records.
Candidates must be a graduate of an accredited School of Nursing and hold a current Registered Nurse license in Georgia, along with at least one year of RN experience; hospice experience is preferred. Applicants must also possess excellent assessment and problem-solving skills, be a licensed driver with an insured vehicle, and be competent in administering pharmaceuticals and performing necessary nursing procedures.

Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around the Rockdale / Dekalb / Newton County, GA and surrounding areas. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Starting Salary: $85,000/yr
