About COREWELL HEALTH VNA Our Visiting Nurses Association (VNA) is the Home Care Team serving West Michigan.
We are seeking a licensed Physical Therapist to join our growing team supporting patients across Holland, Saugatuck, Fennville, and Hamilton.
This role offers a high level of autonomy, allowing clinicians to practice at the top of their license while delivering care in patients’ homes.
Home health physical therapy provides a unique opportunity to help patients regain independence and function in the environment where they live and thrive.
Our therapists maintain manageable caseloads, typically seeing 5–7 patients per day, with thoughtfully designed geographic territories that minimize travel and support work-life balance.

Scope of Work The physical therapist is responsible for examining, evaluating and developing a treatment plan for patients with functional limitations and disabilities.
PT’s provide consultation to healthcare providers regarding a patient’s continuum of care.
The individual in this position is required to appropriately interact with all patients regardless of age or culture and to adhere to the Corewell Health customer service standards.
