The Licensed Practical Nurse will provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes, recording clinical and progress notes. Responsibilities also include coordinating other patient care activities and services to ensure the highest quality of patient care within the assigned geographic service area.
Required qualifications include a current and unrestricted Licensed Practical Nurse licensure in the state of practice, a valid driver's license with vehicle insurance, and access to a dependable vehicle. Candidates must also be able to function effectively in any home situation regardless of the client's background or financial condition.

The hourly pay for this role will range from $21.70 to $32.55 per hour based on full-time employment.
