Nurse Practitioners and Physician Assistants are responsible for assessing patients' medical and mental health needs and developing proactive care plans to manage conditions and address barriers to care. This includes prescribing medications, ordering necessary tests, and monitoring patient compliance and treatment response.
Candidates must possess an APRN or PA license and a degree from an accredited program, along with at least three years of clinical nursing or rehab experience, preferably in geriatric populations or Fee for Service settings. Experience with Electronic Health Records and basic Microsoft Office applications is also required.
