The Substance Use Disorder Case Manager provides comprehensive, recovery-oriented case management services, focusing on care coordination, resource linkage, and advocacy to promote stability and long-term recovery. Essential duties include completing assessments, developing service plans, coordinating with various providers, linking clients to necessary resources, and maintaining compliant documentation.
A minimum requirement is an Associate's degree, with a Bachelor's degree preferred, and an active CDCA Certification is required. Experience working with individuals with substance use disorders or co-occurring disorders is preferred, alongside strong organizational and communication skills.

The Substance Use Disorder (SUD) Case Manager role is a Full-time/Part-time, 1099 contracted roll and provides comprehensive, recovery-oriented case management services to individuals with substance use disorders, including those with co-occurring mental health conditions.
This role focuses on care coordination, resource linkage, advocacy, and ongoing support to promote treatment engagement, stability, and long-term recovery. Services are delivered in accordance with agency policies, Medicaid requirements, and applicable state and accreditation standards. The role is a remote, telehealth position
Telehealth, home office-based, and/or clinical setting. Reliable Internet is required for remote work.
Local travel may be required, Community-based as needed.
Ability to manage caseload expectations and documentation timelines.
Hiring range $21-$26/hr. depending on qualifications and experience.
