The Care Management Clinician provides case management, crisis intervention, and clinical care coordination for children and their families. They are responsible for conducting mental health assessments, developing individualized care plans, and collaborating with interdisciplinary teams to ensure continuity of care.
Candidates must hold a Master's degree in Counseling, Social Work, or Marriage and Family Therapy and possess an appropriate Ohio license. Experience in managing behavioral healthcare for children, adolescents, and families is required, along with a valid Ohio driver's license.
The Care Management Clinician is a key member of a multidisciplinary outpatient team supporting children’s mental health. They provide care management, crisis intervention, family advocacy, and resource coordination, using clinical skills beyond traditional therapy. They collaborate with providers, guide caregivers through systems, and monitor progress to ensure coordinated, high-quality, family-centered care. Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.

Essential Functions: Conducts assessments of patients' mental health needs and develops individualized care plans. Monitors patients' progress and adjusts care plans as needed. Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care. Coordinates available NCH and community resources to enhance client care. Provides education to patients, families, and care teams about mental health conditions and treatment options. Performs brief therapeutic interventions and counseling. Completes clinical charting and other related administrative paperwork in accordance with policy standards. Participates in quality improvement initiatives to improve patient outcomes. Education Requirement: Master's degree in Counseling, Social Work or Marriage and Family Therapy, required. Licensure Requirement: Appropriate Ohio licensure, required.Must remain current in required licensure and clinical or other training as required. Independent license or satisfactory progress toward independent licensure, required.
