The RN Case Manager provides medically-related case management services, including medical care coordination, linkage to health resources, and monitoring referral outcomes for clients in the Housing Services program. This involves delivering health education, performing clinical nursing tasks like assessments and medication adherence support, and conducting independent home/community visits.
Candidates must possess a Bachelor’s Degree from an approved School of Nursing and be licensed as a Registered Nurse in Maryland, along with having a personal vehicle and valid Maryland driver's license. Preferred experience includes two years in case management and two years in clinical nursing, particularly working with individuals with behavioral health disorders.

As a member of the Housing Services team, the RN Case Manager has direct clinical care responsibilities. The Housing Services program was developed for the purpose of helping individuals with behavioral health disorders maintain housing placements. The RN Case Manager’s role is to provide medically-related case management services to clients served by the various Housing Services programs. Within the scope of the RN licensure, the RN will act as a conduit to clients’ medical provider, provide nursing support, and collaborate with the multidisciplinary team in order to improve overall function and produce better housing placement outcomes.
