• QualificationsEducation/Experience:Certificate from college or technical school; three to six months related experience and/or training.

• Registrations:Current Licensed Practical Nurse (LPN ) License for State of Florida Healthcare Provider Cardiopulmonary Resuscitation (CPR) Valid Florida Driver’s License Certification in Hospice and Palliative Care (CHPLN) preferred; required within 2 years of employment
• Computer
• Skills:Strong computer skills including database software and general knowledge of e-mail/Outlook.
• Supervisory
• Responsibilities:This position has no supervisory responsibilities
• Language
• Skills:Communication skills of reading, writing, and speaking in English.
• Mathematical
• Skills:Math skills including addition, subtraction, multiplication, division, and percentages/fractions. Successfully passing a math exam prior to starting orientation and yearly at a skills fair.

Field evaluations involve driving to patient homes. Must be able to endure exposure to common household allergens (i.e., pets, plants, dust).
