The coordinator assesses individual needs, develops goals, and collaborates with Residential Providers to ensure treatment plans are followed. They provide case management services for support, rehabilitation, or crisis intervention, monitoring placements and assisting with necessary legal procedures.
Candidates must be a Qualified Mental Health Professional (QMHP) according to Medicaid guidelines, possessing specialized training or one year of experience working with individuals with mental illness. A minimum requirement is holding a bachelor's degree in a human services field.
