The Hospice RN will evaluate and admit patients to hospice care, develop and implement a plan of care, and provide end-of-life care coordination, including bereavement support. This role also involves acting as a case manager, making necessary adjustments to the care plan, and serving as a liaison between the patient, caregiver, MD, and other team members.
Candidates must possess an Associate's degree in nursing and hold a current Alabama nursing license, along with a minimum of two years of experience in a hospital setting such as MedSurg, Critical Care, or ER. Previous home care or hospice experience is preferred, and strong organizational, critical thinking, and independent decision-making skills are essential.
Associate's degree in nursing and currently licensed in the state of Alabama required.
