The Advanced Practice Nurse will provide comprehensive symptom management services for palliative care patients, initiating consultations, developing treatment plans, and evaluating patient responses. This role involves extensive collaboration with community physicians and participation in interdisciplinary teams, including performing clinical visits and handling night/weekend call duties.
Candidates must be graduates of an approved Advanced Practice Nurse Program with current, unrestricted RN and APN licenses in Ohio and Michigan, preferably specializing in Adult, Family, or Gerontological medicine. A minimum of 3 to 4 years of experience as an APN is required, along with proficiency in EMR systems, CPR certification, and possessing a reliable, insured vehicle.

The Advance Practice Nurse provides timely and comprehensive symptom management services to palliative care patients. The APN will collaborate with the community physicians to implement the plan of care for the patient. The care provided shall be in compliance with regulations established by the Michigan/Ohio State Law and policies of Hospice of Northwest Ohio.
