The Licensed Practical Nurse is responsible for verifying the plan of care with the RN before each visit and collaborating frequently with the physician and RN to ensure patient needs are met, notifying them of condition changes. Key duties also include teaching self-care techniques, checking vital signs, reviewing medications, performing ordered procedures, and completing timely and accurate documentation.
Candidates must possess at least one year of clinical LPN experience and hold a current LPN license specific to the assigned state(s) of work. A valid driver's license, reliable transportation, and liability insurance are also mandatory requirements for this role.
