The Registered Nurse administers nursing care to residents and coordinates the activities of certified nurse assistants. Responsibilities include assessing resident conditions, administering medications, and maintaining accurate medical records.
Candidates must be a graduate of an approved school of nursing with current New York State registration and BLS or CPR certification. Experience in geriatric, medical, and surgical nursing is preferred, along with the physical ability to lift 50 pounds.
The Registered Nurse administers nursing care to designated residents under the direction of Nursing Administration. Coordinates and supports the activities of the certified nurse assistants and actively participates in the direct care of the resident, by following the specific approaches as stated in the plan of care. Evaluates and assigns resident care based on resident need. Schedules and prepares paraprofessionals’ assignments, if needed and supervises same. Inventories controlled drugs at the beginning and end of each shift and follows established policies and procedures. Administers prescribed medications and treatments. Reorders medications and treatment supplies as needed for assigned team. Performs ROM exercises for assigned residents in the absence of the Rehab Aide. Assures nursing rehab has occurred as indicated and is documented. Assess residents’ condition daily. Reports changes in condition to Coordinator/ Supervisor including, but not limited to, reaction/effectiveness of medication and treatments, changes in physical or mental status, or significant incidents. Documents all such findings, completes incident reports and updates care plans, as needed. Maintains residents medical record according to facility policy. To include, but not limited to, medication and treatments given, I&O, vital signs, tests weights, physical, mental, and emotional status, and completion of weekly skin assessments daily. Instructs, assists, and supervises nursing assistants in rendering nursing care on the unit. Maintains ongoing communication with nursing assistants throughout the shift and receives end-of-shift report from same. Assesses completion of the nursing assistant assignments and appropriate documentation on the restraint sheets, toileting sheets and BM records. Makes periodic rounds during shift and reports significant problems to coordinator or supervisor. Admits and orients new residents to the unit by completing initial assessment of residents’ condition, skin, care needs, etc. and documents such utilizing the nursing assessment form. Assists in initiating the resident plan of care. Completes other required documentation i.e.: tube feeding flow sheet, glucometer records etc. Supervises dining areas and assists with feeding residents. Completes Meal Acceptance Sheet daily. Supervises nourishment pass and documentation. Answers resident call lights to determine resident needs and performs the service or directs them to a paraprofessional for performance of service. Accompanies physicians on rounds and assists with exams and treatments, when necessary. Receives and transcribes physician orders. Completes nightly medication checks (11-7 nurse). Requisitions supplies, equipment, linen, etc. to meet unit needs. Checks to see that the unit is clean and functioning properly. Maintains equipment according to established policy and procedure. Checks in pharmacy order for unit, if necessary. Completes 24-hour report. Gives report at end of shift, if required. In case of fire/disaster or fire/disaster drill, assures that the safety of residents and staff is maintained and that the fire/disaster procedures are properly carried out. Participates in employee evaluations, recommends, or initiates disciplinary action according to the progressive discipline policy established by Niagara Lutheran Health System. May assume responsibilities of nurse in charge under the supervision of the registered nurse supervisor. Adheres to established policies and procedures of Niagara Lutheran Health System. Assists in the orientation of new staff. May be designated as charge nurse on the 3-11 or 11-7 shift assuming responsibility for overall management of nursing unit under the supervision of the RN supervisor. May perform other duties as directed by the nurse in charge, coordinator or supervisor. Always adheres to dress code policy. Always adheres to time and attendance policy. Assures the safety, security, and wellbeing of our residents. nlpp QualificationsPosition Requirements – RN Staff Nurse EXPERIENCE: Experience preferred in geriatric, medical and surgical nursing. EDUCATION REQUIREMENTS: Graduate of an approved school of nursing. Current registration in New York State is required. LPN’s must have CPR training. RNs must be BLS or CPR certified. SPECIFIC REQUIREMENTS: Acute sense of responsibility, integrity, and loyalty. Ability to deal with residents under all conditions. Sincere desire to work with the aging and infirm. Ability to anticipate moods and needs of geriatric residents. Compassion and patience essential. Pleasant, cheerful, level-headed, courteous when handling residents, relatives, and co-workers. Ability to function in a professional manner during emergency situations. Ability to treat resident information as confidential. Ability to cooperate with other personnel and to supervise when necessary. Neat, well-groomed appearance. PHYSICAL AND SENSORY REQUIREMENTS: Must have the physical ability to lift a minimum of 50 pounds. Must submit to and pass a yearly examination, PPD skin test and attend all mandatory inservices. Must have the ability to push, pull and bend throughout the day. WORKING CONDITIONS: Stands, sits and walks most of shift. Subject to many interruptions and changes of priorities. Periodic contact with residents, employees and families. Required to make decisions within the established guidelines. Required to handle emergency situations. Well-lit, well-ventilated nurses’ station, resident rooms and halls. Exposed to infectious odors, peculiarities, and mood changes of aging persons. Frequent heavy lifting and moving of residents. Must share weekend and holiday work.
