The Nurse Practitioner will provide comprehensive mental health and cognitive care services to residents in independent and assisted living communities. This includes developing evidence-based treatment plans and collaborating with a supportive clinical team.
Candidates must be an Adult-Gerontology Nurse Practitioner (AGNP) or Family Nurse Practitioner (FNP) with a passion for the geriatric population and confidence in treatment planning. Strong knowledge of pharmacology and the ability to work independently while collaborating are essential.

The iTrust Cares division of iTrust Wellness Group provides comprehensive mental health and cognitive care services to residents of independent and assisted living communities throughout South Carolina. As we continue to expand our presence in the Lowcountry, we are seeking a skilled and compassionate Adult‑Gerontology Nurse Practitioner (AGNP) or Psychiatric‑Mental Health Nurse Practitioner (PHMP) to join our team in a contract‑to‑hire capacity as an independent contractor.
This role begins as a 1099 independent contractor position, with the opportunity to transition into a permanent role based on mutual interest, performance, and organizational needs.
