The Nurse Practitioner is responsible for coordinated and comprehensive care of members at designated skilled nursing facilities, collaborating with primary care physicians and the interdisciplinary team. They provide preventive care, diagnose illnesses, order tests, prescribe medications, and establish care plans.
Candidates must be currently licensed as a Nurse Practitioner and have a minimum of 3 years of clinical care experience, preferably in Skilled Nursing Facility or Long Term Care. Additional preferred qualifications include case management experience and knowledge of I-SNP MOC.
Job Summary: Nurse Practitioner
The Institutional Special Needs Plan (ISNP) Nurse Practitioner is responsible for coordinated and comprehensive care of its Provider Partner Health Plan (PPHP) members at designated skilled nursing facilities.
The Nurse Practitioner works in collaboration with the member’s primary care physician to deliver exception health care. The nurse practitioner is the medical liaison to the interdisciplinary staff and allied health providers and communicates effectively to assure seamless delivery of services.
As a key member of the clinical team in the skilled nursing facility, the Nurse Practitioner provides preventive care, diagnoses both acute and chronic illnesses, orders and interprets diagnostic tests, prescribes medications, refers to specialists as appropriate, establishes a plan of care with short-term and long-term goals, and is a resource to members, families, and staff.
