The Float Advanced Practice Provider conducts comprehensive patient assessments and manages health conditions across multiple clinic locations. They collaborate with interdisciplinary team members to ensure seamless care delivery.
Candidates must have a Master's in Nursing or Doctor of Nursing Practice and a minimum of 2-3 years of Nurse Practitioner experience in a primary care setting. Strong clinical assessment and communication skills are essential.

Conduct comprehensive patient assessments, including history taking and physical examinations across assigned clinic sites. Diagnosis and manage acute, chronic, and preventative health conditions in alignment with evidence based guidelines. Develop and implement patient care plans, including ordering and interpreting diagnostic tests, prescribing medications and initiating referrals as deemed appropriate. Maintain accurate, timely, and thorough documentation in the electronic medical record (Cerner). Collaborates with providers, care coordinators, medical assistants, and other interdisciplinary team members to ensure seamless care delivery. Participate in shared call with primary care providers.
Completion of Masters in Nursing or Doctor of Nursing Practice from an accredited program Minimum of 2-3 years of Nurse Practitioner experience within a primary care setting Strong clinic assessment, critical thinking, and decision making skills Excellent communication and interpersonal skills with patients and care team members The above statements are intended to describe the functions and related requirements of persons assigned to this job.
