Team Nurse (RN) (Behavioral Health Services)

• Overview We are seeking a new PACT Registered Nurse for our Program for Assertive Treatment (PACT) teams to provide treatment to adults with persistent mental illness.
• The PACT Registered Nurse will work on an interdisciplinary team of nurses, clinicians, and outreach workers to assist those in need in the community.
• The PACT Registered Nurse will work under the supervision of the Lead Nurse to conduct psychiatric assessments in addition to physical health needs in order to provide holistic nursing services.
• The PACT Registered Nurse will be responsible for making appropriate referrals to community physicians, providing management and administration of medication in addition to providing a range of treatment, rehabilitation, and support services.
• Other responsibilities include but are not limited to: Providing case management for an assigned group of persons served, including coordinating and monitoring the activities of the individual treatment team Conducting a comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health Providing ongoing assessment, problem solving, side-by-side services, skill training, supervision, and environmental adaptations to assist persons served with activities of daily living Keywords: VinfenRN, VinfenPACT
