The pharmacist will order and interpret patient-specific data, assess medical problems, evaluate drug therapy, and develop therapeutic plans. They will also educate patients and caregivers about medication treatment plans and consult with other healthcare providers to improve patient outcomes.
Candidates must have a doctoral degree in Pharmacy and a current, active license to practice pharmacy. Additionally, one year of professional pharmacy experience is required, including comprehensive medication management and patient education.
Overview Pharmacist (Clinical) - Fort Carson, Colorado
MAJOR DUTIES Order and interpret patient-specific data, assesses medical problems, evaluate current drug therapy, develop therapeutic plans, monitor drug therapy, and dispense or administer medications or therapies as needed or indicated. Promote cost-effective prescribing through use of facility drug formulary and optimizing use of non-formulary medications when needed. Conduct patient and family-centered or caregiver education related to the medication treatment plan, expected outcomes, potential adverse effects, as well as in the preparation, storage, handling, and administration of medications where appropriate. Consult with other patient care providers to improve patient outcomes. Advise on the safe and effective use of drug treatments and alternatives. Develop medication use evaluations and evidence-based prescribing guidelines to assist providers in safe and appropriate medication use Performs other duties as assigned.
Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. A Personnel Security Investigation is required. Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption. This position is a Testing Designated Position subject to pre-employment screening and random drug testing. Selectees will be required to consent to participation in random drug urinalysis testing. A pre-placement medical examination is required. Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position. Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross or American Heart Association. Advanced certification (e.g., ALS) does not supersede BLS. Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
