As a Home Care Nurse, you will oversee the quality of care for assigned homecare patients, administer medications, and assist clients in achieving their care goals. You will also document care provided and collaborate with the clinical team to improve care plans.
Candidates must have a valid State LPN/RN License and a valid driver's license due to travel requirements. They should also be able to read and understand various agency documents and promote excellent customer service.
What You’ll Do: ∙Administer medication in accordance with standards of practice. ∙ Encourage social\rehabilitative skills and encourage self-care and independence. ∙ Perform client treatments in accordance with Plan of Care ∙ Assist the consumer in learning appropriate self-care techniques. ∙ Prepare clinical and progress notes to document care provided. ∙ Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor ∙ Promote activities to achieve a safe and healthy environment. ∙ Observe aseptic technique when performing client procedures/care. ∙ Collaborate with other members of the clinical team to develop and improve the Plan of Care ∙ Assist the client in achieving established goals in the Plan of Care developed by the RN. ∙ Maintain productivity according to the standards of the agency. ∙ Maintain license, CEUs and mandatory employment requirements. ∙ Participate in client care meetings, in-services, orientation and staff meetings. ∙ Provide service in accordance with agency’s philosophy, standards, policies and scope of practice ∙ Participates in Performance Improvement and adheres to HIPAA guidelines. ∙ Other duties as assigned.

Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures. Must be able to use a computer, laptop or tablet. Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
