Responsibilities

What You Will Do: Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits Conduct evaluations and develop occupational therapy treatment plan for our patients Deliver treatment/care in accordance with care plan Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications
