Coordinate interdisciplinary care plans for patients from admission to discharge, integrating family preferences and values. Advocate for necessary resources and maintain communication with healthcare providers to ensure effective health plan implementation.
Requires a professional credential as a Social Worker, Clinical Counselor, or Therapist from the Illinois Department of Financial and Professional Regulation. Education requirements include a high school diploma with 2 years experience, an associate degree, or 4 years of applicable job-specific experience.

How you’ll make an impact in this role Coordinate the interdisciplinary plan of care across the continuum from admission to discharge Integrate patient and family preferences while monitoring care through assessments and records Advocate for resources, remove barriers, and maintain ongoing dialog with supervisors and providers Act as a resource for documentation accuracy, service appropriateness, and post-discharge options
One or more of the following required: Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
