The Hospice RN Case Manager administers skilled nursing care to terminally ill patients and coordinates overall care for their caseload. This includes making regular visits, assessing patient needs, and communicating with the interdisciplinary team.
Candidates must be a graduate of an accredited nursing program and hold a current state license as a Registered Nurse. A minimum of one year of clinical nursing experience is required, with hospice or home health experience preferred.

Pay Range $78,000—$85,000 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
