The Advanced Practice Provider conducts monthly in-person visits to healthcare facilities and manages between 1 to 12 Tapestry affiliated facilities. They deliver and review various reports with facility staff and facilitate discussions to enhance patient care.
Candidates must be licensed nurse practitioners in Florida, with preferred experience in chronic care management, remote monitoring, or behavioral health integration. Strong interpersonal and communication skills are essential for effective collaboration with medical staff.

Tapestry Management Providers (TMPs) play a crucial role in delivering comprehensive, in-person clinical support to healthcare facilities. They ensure proactive patient care through the review and discussion of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI) & daily risk reports with facility healthcare professionals, including physicians, nurse practitioners, and social workers.
The anticipated salary range for this role is $125K-135K annualized. This hybrid position follows a location-based compensation structure. The posted salary range represents the potential pay range across the geographic markets. Actual compensation will be determined based on the candidate’s experience, qualifications, and internal equity considerations, in accordance with applicable pay transparency laws.
