The Social Worker will conduct comprehensive psychosocial assessments and collaborate with interdisciplinary teams to develop individualized care plans for residents. They will also provide counseling, crisis intervention, and advocacy to ensure the holistic needs and rights of residents are met.
Candidates must hold a Master’s degree in social work and possess current licensure or certification in the state of employment. Previous experience in long-term care or geriatrics is required, along with strong assessment and interpersonal skills.
