The Care Coordinator will assess the needs of children and families to develop and monitor individualized care plans using the Wraparound model. They will also facilitate team meetings, provide advocacy, and coordinate with local service providers and schools to ensure access to necessary resources.
Candidates must possess a Bachelor’s degree in a Human Services related field and have at least two years of experience in a mental health setting. A valid driver’s license is required, and the ability to obtain CPR/First-Aid certification within six months of hire is mandatory.

About the program: Child Outpatient Program
Psychiatric and diagnostic evaluation and treatment is provided to children, adolescents and families. Our child and adolescent offices are designated as Enhanced Care Clinics under Husky, and offer integrated behavioral health services. Individual, family treatment, psychiatric evaluation, medication management, MATCH, ARC, and TF-CBT (Trauma Focused Cognitive Behavioral Therapy) are available.
About the Position: Care Coordinator
Bachelor’s Degree in Human Services related field required.
Minimum of 2 years' experience in mental health setting.
Valid driver’s license. CPR/First-Aid Certification within 6 months of hire.

Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
