Provide high-quality, patient-centered care to residents in skilled nursing facilities through daily rounding and treatment planning. Maintain reliable on-site attendance while ensuring compliance with clinical standards and facility policies.
Candidates must hold an active U.S. license and be board certified as a Nurse Practitioner or Physician Assistant. Qualified specialties include Family Medicine, Internal Medicine, Geriatric Medicine, or related fields.

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
