Assess patients in clinical and contracted facilities while completing medical histories and physical examinations. Accurately document care plans in the electronic medical record and assist the care team in meeting performance measures.
The candidate must be a licensed Nurse Practitioner or Physician Assistant capable of providing care within their scope of practice. Proficiency in electronic medical record systems and adherence to clinical guidelines are required.
Assess patients in clinic and other outside facilities that are contracted with SMG/SRH, and completes medical histories and physical examinations to appropriately provide care to patients in accordance with the medical plan of care and within own scope of practice. Accurately chart history and care plan in electronic medical record (EMR), closing encounters within the time guidelines set forth by Self Medical Group and Self Regional Healthcare Administration. Completing all EMR generated tasks and assisting care team in achieving Meaningful Use measures. Performs all other assigned duties per SMG Physicians, Practice Manager, and Self Medical Group Administration.
