The Case Manager provides therapeutic support to patients while coordinating aftercare plans and discharge recommendations with the clinical team. They also maintain communication with referral sources and facilitate group therapy sessions in accordance with regulatory standards.
Candidates must hold a bachelor's degree in behavioral sciences, counseling, or a related human services field. A minimum of 1 to 2 years of experience in mental health settings is required, with a preference for certified behavioral health case managers.

The Case Manager provides a strong therapeutic relationship to the patient, collaborating with the Primary Therapist in identifying, establishing and communicating the clinical recommendations for continued care. Case Manager will work with the patient coordinating the aftercare/continuing care recommendations provided by the clinical team. The Case Manager will provide support to the patient in respect to legal and any other identified documentation. Case Manager is to maintain ongoing communication and relationships with referral sources as well as invested parties.
