Requires a Bachelor's degree in Psychology, Social Work, or a related field. Must possess a valid Georgia Driver's License, a clean MVR, and pass a criminal background check.

Clarvida is seeking an ACT (Assertive Community Treatment) Case Manager to support adults experiencing severe mental health challenges. In this role, you will partner with clients to develop and implement personalized service plans, focusing on areas like housing, employment, therapy, medical care, and substance use support. You will work directly with clients in their communities to improve their quality of life and facilitate their recovery journey. As an ACT Case Manager, you will collaborate with a dedicated team, provide hands-on support for daily living skills, and connect clients to necessary resources. You will also help track progress through home visits, offer transportation, and contribute to team-based client goals.
● Collaborate with clients to create and implement personalized service plans ● Provide hands-on support in daily living skills, including budgeting, grocery shopping, meal planning, social skills, and coping strategies ● Connect clients to community resources and services ● Conduct regular home visits to track progress and offer support ● Collaborate with the ACT team to help clients achieve their goals ● Provide transportation as needed ● Rotate on-call responsibilities every 6 weeks (virtual or in-person)
