Provide after-hours phone triage and support for hospice patients and their families. Travel to patient homes to deliver care for symptom management and end-of-life support as needed.
Must hold an active North Carolina RN license and possess strong clinical judgment. Experience in hospice or home health is preferred, along with the ability to work independently.

This weekend position is on call from home, but it is not fully remote. You must be available to travel for in-person visits when patient needs arise—including urgent symptom management and end-of-life care.
In this role, you will provide phone triage and support, travel to patient homes when needed, and deliver care for symptom management, active decline, and at times, support patients and families at the end of life—including during and immediately after death. These are often the moments when your presence matters most.
